The Current Landscape


Now that words like ‘telehealth’ and ‘remote patient monitoring’ are part of

common vernacular, FormDr industry has vast changes happening, and fast. Secure, digital intake became a nice to have for their customers to offer to their customers, but after March of 2020, it became a necessity. With the boom of digital health tech, also came more options, more customers coming into the market, and customers outside of the health space now interested in the security that comes with HIPAA compliance.


There are companies that also offer digital documents, like DocuSign or Adobe, that many everyday users are very familiar with. We are competitors because they all offer digital forms, however, FormDr does take it a level deeper by offering full security and HIPAA compliance, plus the added benefit of knowing our product was created with a niche of customers in mind, not just everyone.


Company Birth Story


The idea was conceived during a pivotal moment in our founders' career

while heading up lead interface design at the US Department of Defense. It was the fusion of good design and the utmost in cybersecurity - the seed was planted realizing the importance of security but also didn't want to let go of the equal necessity of captivating design.


However FormDr wasn’t birthed until our founder was working at his last company, he found their B2C customers were asking for a service that wasn’t provided - a more secure way to save and collect end customer information. It was then the idea was put together to build them something that was secure and offer them something that would not only do intake but optimize their whole process, it was an ah-ha moment when the idea of FormDr was conceived.


The Solution


FormDr has built a system to streamline and simplify cumbersome

paperwork flows that have been a mandatory part of everyday office life. The aim is to give a better experience to the end patient, and through the adoption of FormDr, give these patients a better representation of the service/care they will be provided. Ease of care from the very start. Understanding the medical atmosphere often can be highly stressful and scary environment, they aim to ease that by making the transfer of information seamless, accurate but simple.


And how do FormDr compare? By offering the best design quality digital forms available. Patients love their forms, they are more intuitive and it's beyond easy to complete. A product can be a great idea but if it's not easy to use or adopted, it's obsolete. This product was created with “on the go” people in mind. So there are no user accounts necessary to fill out a patient form, optimized for “Mobile-friendly online intake forms that your patients complete from any device, anywhere.” No pinching to zoom or not understanding what to click next. They understand people need a person's help, particularly when it comes to new tech. That's why they have support and training as a core feature, not an afterthought.


Success can be measured differently but to them, it can be defined as “21 minutes”. Throughout customer surveys, they’ve calculated that 21 minutes of staff/administrative time is saved per patient on average. With 10 million+ new patient intake submissions, that's 210,000,000 minutes saved, which equals 3.5 million hours given back to office administrators, doctors, nurses, PTs,hygienists, therapists, front office staff to give better care, take a lunch break, time off to be with their family, do research on new treatments and so much more.


Looking forward, the streamlining doesn’t stop with intake. FormDr aim to streamline the rest of what goes into the administrative side of medical care to optimize the time of these caregivers and allow them the ability to choose what they do with it.


A Customer Story


In the February 2021 issue of Optometric Business Review , Dr. Keshav

Bhat revealed some of the ways new technology is helping to build and improve his optometry practice. Like most medical practices, of any type, Dr. Bhat talks about how “paperwork has been a thorn in our side for years.” Most practice owners could agree with this statement! With new regulations added seemingly every year, the amount of paperwork required from each patient grows.


Dr. Bhat’s clinic was no different than others. Patient forms had to be filled out for each visit, then were scanned into the EMR – and then went straight into the paper shredder. When the pandemic began, one change Dr. Bhat’s practice made was to mandate that all necessary forms be moved online to where those could be completed on the clinic website prior to the visit, making most fields required to ensure accuracy. This minimized any risk of viral transmissions by touching of papers and pens, helping to maintain a clean and safe environment. It also resulted in fully completed, accurate records.


Dr. Bhat used FormDr. to embed online medical form templates on the office website and enjoyed the smooth transition. He has been pleased with the ease of use, more sanitary work environment, and environmental impact of forgoing all those wasted, unnecessary paper printouts!

This gave Dr. Bhat the opportunity to discuss preventative healthcare, nutrition, and other topics.
